Student Vaccination Survey Hello JCSD Family,JCSD is partnering with Coastal Carolina Hospital to make the Pfizer vaccine available to all students 12 years old and older. We will host a student vaccination event ahead of the 2021-2022 school year. The purpose of this survey is to get a count for how many students are interested in receiving the vaccination. Please fill out a survey for each student that you would like to receive the vaccination. You can access the survey at the link below.
